Passport applications can be obtained and submitted through the Salt Lake County Clerk’s Office, Passport Division. This application is also available online.

General Passport Information:
- An adult passport (16 years of age and older) is valid for 10 years.
- A child’s passport (ages 0-15 years old) is valid for 5 years.
- DO NOT sign the passport application until requested.
- NOTE: As of March 6, 2007, women using a former spouse's surname are required to provide a certified copy of the previous marriage license.

NOTE: The best advice is, "APPLY EARLY!"
- Refer to the US Department of State Website for the most up to date processing times.
Proof of Citizenship:
- If born within the United States you must provide a certified copy of your birth certificate. (ITEMS NOT ACCEPTABLE include: hospital certificate, baptism, or notification-of-birth and California and Texas abstracts of birth.)
- If born outside of the United States you must provide a certificate of citizenship or naturalization, or report of birth abroad.
Personally Appear:
- ALL APPLICANTS must personally appear when applying for a passport.
- Children under 16 years of age must have both parents present.
- Children 16 years of age and older must have one parent present.
- Social security number must be provided for all passport applicants!
Passport Photograph Requirements:
- Passport photographs are available in the Clerk’s Office for ages 5 years and older for a $10.00 fee. (cash, check, money order, MasterCard or Visa)
- Two identical U.S. passport photographs are required.
- Passport photographs must have been taken in the last 6 months.
- Pictures must be in color and on a white or light colored background.
- Pictures must be two-inches square. (Image size of head must be between 1 inch and 1 3/8 inches including hair.) See a qualified passport photographer.
Identification Requirements:
- Valid driver's license with current name. (NOTE: Temporary driver's licenses and driving privilege cards are not acceptable.)
- Federal/State identification card with current name (temporary identification is not acceptable.)
- Both parents must appear and sign for a minor applicant and present current identification and proof of relationship to applicant (such as birth certificate when minor is under the age of 14.)

16 years or older:
Total $100.00 (two separate payments required)
- $75.00 (U.S. Department of State, check or money order only)
- $25.00 (make check payable to: Office of Sherrie Swensen) (cash, check, money order, MasterCard or Visa)
15 years or younger:
Total $85.00 (two separate payments required)
- $60.00 (U.S. Department of State, check or money order only)
- $25.00 (make check payable to: Office of Sherrie Swensen) (cash, check, money order, MasterCard or Visa)
Passport Card:
- Passport cards will facilitate entry and expedite document processing at U.S. land and sea ports-of-entry when arriving from Canada, Mexico, the Caribbean and Bermuda.
- The passport card may not be used to travel by air.
